An MRI MRCP Plain (Magnetic Resonance Cholangiopancreatography) is a specialized, non-invasive MRI examination designed to produce detailed images of the biliary and pancreatic ductal system without using contrast material. Using advanced magnetic resonance imaging (MRI) technology, MRCP provides high-resolution visualization of the liver, gallbladder, bile ducts, common bile duct (CBD), pancreatic duct, and pancreas, making it one of the most accurate imaging techniques for diagnosing disorders affecting the hepatobiliary system.
Unlike conventional imaging procedures, MRCP does not require endoscopy or ionizing radiation. It allows doctors to evaluate the flow of bile and pancreatic secretions while identifying blockages, stones, strictures, congenital abnormalities, inflammation, and tumors involving the biliary tract and pancreas. Because of its excellent soft tissue contrast, MRCP has become the preferred non-invasive alternative to many diagnostic endoscopic procedures.
Doctors commonly recommend an MRI MRCP Plain for patients experiencing persistent upper abdominal pain, jaundice, abnormal liver function tests, suspected gallstones within the bile ducts, pancreatitis, unexplained bile duct dilatation, or suspected pancreatic and biliary diseases. It is also frequently used before surgery and for monitoring chronic hepatobiliary conditions.
At Jinnah MRI, Lahore, MRI MRCP Plain examinations are performed using advanced high-field MRI systems that provide exceptional image quality and detailed visualization of the hepatobiliary system. Every examination is interpreted by experienced consultant radiologists to support accurate diagnosis and effective treatment planning.
Clinical Procedure: What to Expect at Jinnah MRI
MRI MRCP Plain is a painless, non-invasive imaging examination performed without contrast injection.
Patient Preparation
- Fast for 4 to 6 hours before the examination unless instructed otherwise.
- Avoid eating fatty meals before your appointment.
- Drink only small amounts of water if necessary during the fasting period.
- Remove all metallic objects, including jewelry, watches, belts, hearing aids, coins, and mobile phones.
- Inform the MRI staff if you have a pacemaker, cochlear implant, aneurysm clips, implanted medical devices, or metallic foreign bodies.
- Wear comfortable clothing or change into a hospital gown if required.
- Bring previous ultrasound reports, CT scans, MRI scans, ERCP reports, laboratory investigations, or surgical records for comparison if available.
During the MRI Scan
- You will lie comfortably on the MRI table while your upper abdomen is positioned inside the scanner.
- The technologist may ask you to briefly hold your breath during certain imaging sequences to improve image quality.
- It is important to remain completely still throughout the examination.
- The MRI scanner produces rhythmic tapping sounds, and ear protection may be provided.
- The examination usually takes 20 to 30 minutes, depending on the imaging protocol.
When is an MRI MRCP Plain Performed?
Suspected Gallstones in the Bile Duct
MRCP is commonly performed to detect common bile duct (CBD) stones that may cause pain, jaundice, or infection.
Evaluation of Jaundice
Doctors recommend MRCP to identify biliary obstruction, strictures, tumors, or congenital abnormalities responsible for obstructive jaundice.
Investigation of Pancreatic Disorders
The examination helps evaluate acute pancreatitis, chronic pancreatitis, pancreatic duct abnormalities, pancreatic cysts, and pancreatic tumors.
Assessment of Biliary Tract Disorders
MRCP accurately evaluates bile duct strictures, congenital biliary abnormalities, primary sclerosing cholangitis, and postoperative biliary complications.
Abnormal Liver Function Tests
Patients with unexplained elevations in liver enzymes or cholestatic liver function abnormalities may undergo MRCP to identify obstruction or biliary disease.
What Does an MRI MRCP Plain Detect?
An MRI MRCP Plain helps diagnose or evaluate:
- Common bile duct (CBD) stones
- Gallstones affecting the biliary tract
- Biliary obstruction
- Bile duct strictures
- Dilated bile ducts
- Acute pancreatitis
- Chronic pancreatitis
- Pancreatic duct obstruction
- Pancreatic cysts
- Pancreatic tumors
- Cholangiocarcinoma
- Primary sclerosing cholangitis
- Congenital biliary abnormalities
- Post-surgical biliary complications
- Hepatobiliary disorders

MRI MRCP Plain Findings Overview
| Structure Evaluated | Normal Findings | Possible Abnormal Findings |
| Gallbladder | Normal size and appearance | Gallstones, inflammation, sludge |
| Common Bile Duct | Normal diameter without obstruction | CBD stones, strictures, dilatation |
| Intrahepatic Bile Ducts | Normal caliber | Dilatation, obstruction, congenital abnormalities |
| Pancreatic Duct | Normal size and course | Dilatation, obstruction, pancreatitis |
| Pancreas | Normal signal and morphology | Pancreatitis, cysts, tumors |
| Liver | Normal appearance | Biliary obstruction, associated liver disease |
| Surrounding Soft Tissues | No abnormal findings | Masses, inflammation, postoperative changes |
Note: MRI MRCP findings should always be interpreted by a qualified radiologist together with the patient’s symptoms, physical examination, laboratory investigations, liver function tests, and previous imaging studies. Additional investigations such as ERCP, endoscopic ultrasound (EUS), or contrast-enhanced MRI may be recommended when clinically indicated.
